ADA diet plans, i need one for my clinic?

Can someone tell me a website where I can download a diabetic diet plan complete with sample meals and lists of foods for diabetics. I would like to have one that covers from 1200 to 2000 calories. I am a physician and need a patient handout for my new practice.
